At Associate Chiropractic Clinic, we understand that being involved in a Car Accident or "Motor Vehicle Accident" (MVA) can be a stressful and confusing experience. There are many important and time sensitive things to do... but highest priority being your health. That is why we do our very best to simplify the process and make it as simple as possible. We communicate with your Insurance Injury Adjuster, and take care of all the paperwork for you. Our Doctors and Staff at Associate Chiropractic are here to help after a Car Accident.
-
Most patients are entitled to fully covered treatments after a Car Accident (MVA). A maximum amount of 21 treatments within protocol, but they must be used within 90 days of the accident.
-
First an AB-1 Proof of Loss form must be completed. Your Adjuster may send you one or we can provide you with one at our clinic.
-
Second, the Chiropractor will assess you and complete a form called an AB-2 Treatment Plan.
-
We will submit all of your forms for you and keep copies on file if you ever need them.
-
Last, your Injury Adjuster will send you a letter of approval under the Diagnostic and Treatment Protocols. We will bill your insurance company directly for the first 21 treatments (or 90 days, whichever comes first).
-
After 21 treatments or 90 days is reached, we will submit an AB-4 Concluding Report on your behalf to your insurance adjuster stating that you have met your goals and returned to pre-accident status, or you need more treatment and require an extension to your treatment plan. Your appointments are now considered outside of protocol also known as "Section B". In most cases, we may still be able to direct bill to your Insurance Injury Adjuster, however you will need to use your own health benefits first.
-
Your Chiropractor will reassess and create a new Treatment Plan.
-
The insurance adjuster will send you a letter/email indicating if you have been approved or not.
-
If at that time you still require additional treatment because you are not yet at pre accident status additional steps will be advised by your doctor.
We Will Work With Your Insurance to Help You
-
After the initial 21 visits you are considered in Section B or "Out of Protocol"
-
There are government-regulated maximums for some services, regardless of the number of treatments you have been approved:
-
Chiropractic Care $1000
-
Massage Therapy $350
-
Acupuncture $350
-
-
You have up to 2 years to use these benefits with an active treatment plan.
-
Subject to change if the Insurance Injury Adjuster decides to discontinue care.

At the Associate Chiropractic Clinic, our mission statement is:
“When you entrust us with your health, 2 things are at stake – your well-being and our reputation. We wouldn’t do anything to damage either one.”
